About ATM

ATM encodes a kinase that acts as a master controller of the cellular DNA damage response: it detects double-strand breaks and coordinates DNA repair, cell cycle arrest, or apoptosis through downstream targets including p53, BRCA1, and CHK2. Loss-of-function mutations in both copies of ATM cause ataxia telangiectasia, an autosomal recessive disorder characterized by neurological problems, immune deficiency, and cancer predisposition. ATM functions as a tumor suppressor: individuals with ataxia telangiectasia are prone to childhood lymphomas, leukemias, and breast cancer, while carriers of a single altered copy have increased risk of breast, pancreatic, prostate, and other cancers. Somatic ATM mutations also occur in lymphoid malignancies and solid tumors, and ATM-deficient cancers are often especially sensitive to DNA-damaging treatments.

Met · 3

PVS1 Very strongMet: c.1039G>T creates a premature stop at residue 347 of the 3057-residue ATM protein, upstream of the final coding exon and predicted to trigger nonsense-mediated decay. Details ▾Hide ▴
Reasoning

ATM c.1039G>T is a nonsense variant, p.(Glu347Ter), in exon 8 of the MANE Select NM_000051.4 transcript. It introduces a premature termination codon at residue 347 of the 3057-residue reference protein, far upstream of the final coding exon, and is therefore predicted to undergo nonsense-mediated decay rather than produce a distal truncated protein. The ATM HBOP VCEP specifies PVS1 for null variants in ATM, identifies NM_000051.3 as the default transcript with constitutive exons, and cautions only for extreme 3′ LOF variants or splice variants with in-frame exon skipping; neither caveat applies. Full-strength PVS1 is appropriate.

Rule
ATM HBOP VCEP v1.5 PVS1 decision guide: apply PVS1 to a null variant in ATM when loss of function is a known disease mechanism; use caution only at the extreme 3′ end or for splice variants predicted to cause in-frame exon skipping with the remainder of the protein intact.
Evidence considered
  • Variant normalization identifies NM_000051.4:c.1039G>T as NP_000042.3:p.(Glu347Ter), a nonsense change in exon 8; the reference protein is 3057 residues and the predicted protein terminates at residue 347.
  • The ATM HBOP VCEP PVS1 guide defines PVS1 for nonsense, frameshift, canonical splice, initiation-codon, and exon-deletion null variants in ATM, states that the default NM_000051.3 transcript exons are constitutive, and limits its stated cautions to extreme 3′ LOF variants and splice variants predicted to skip an exon in-frame while leaving the remainder intact.
  • The gene-level PVS1 eligibility assessment identifies ATM loss of function as an established germline disease mechanism under the applicable VCEP framework.

PM2 SupportingMet: gnomAD v4 aggregate allele frequency 0.000186% is below the PM2 Supporting cutoff of 0.001%. Flagged for human review: the Admixed American subpopulation frequency (0.005%) exceeds the cutoff. Details ▾Hide ▴
Reasoning

The ATM VCEP v1.5 PM2 Supporting threshold is frequency <=0.001% in gnomAD v4. The aggregate gnomAD v4 allele frequency is 1.858980237800752e-06 (0.000185898%), which meets this threshold, so PM2 Supporting is met. However, the maximum observed subpopulation frequency is 4.998833605492052e-05 (0.0049988%) in Admixed American individuals, above the stated PM2 cutoff; this overall-versus-subpopulation threshold discrepancy warrants human review rather than being silently resolved.

Rule
Frequency <=0.001% in gnomAD v4 dataset; if n=1 in a single subpopulation, PM2 Supporting applies
Evidence considered
  • ATM VCEP v1.5 specifies PM2 Supporting at frequency <=0.001% in the gnomAD v4 dataset and notes that n=1 in a single subpopulation is sufficient for PM2 Supporting.
  • gnomAD v4 reports total AF = 1.858980237800752e-06 (0.000185898%), based on 3/1,613,788 alleles, with 0 homozygotes.
  • The highest observed subpopulation is Admixed American: 3/60,014 alleles, AF = 4.998833605492052e-05 (0.0049988%), with 0 homozygotes; grpmax FAF = 1.327e-05.
Still missing
  • The VCEP PM2 wording states a frequency threshold but does not explicitly state whether the aggregate AF or the maximum subpopulation AF must control when they differ.

PM5 SupportingMet: the premature stop p.(Glu347Ter) lies upstream of p.Arg3047, meeting the ATM VCEP PM5 supporting rule for truncating variants. Details ▾Hide ▴
Reasoning

The ATM VCEP v1.5 PM5 specification explicitly permits PM5 at Supporting strength for frameshifting or truncating variants with premature termination codons upstream of p.Arg3047. NM_000051.4:c.1039G>T produces the premature termination codon p.Glu347Ter, which is upstream of p.Arg3047; the criterion is therefore met at Supporting strength.

Rule
ATM VCEP v1.5 PM5: apply to frameshifting or truncating variants with premature termination codons upstream of p.Arg3047; Supporting strength is specified for this pathway.
Evidence considered
  • The case normalization identifies NM_000051.4:c.1039G>T as the truncating variant NP_000042.3:p.(Glu347Ter), at residue 347.
  • The ATM VCEP v1.5 PM5 rule states: apply to frameshifting or truncating variants with premature termination codons upstream of p.Arg3047.

BA1 Not met: gnomAD v4 grpmax filtering allele frequency 0.001327% is far below the 0.5% BA1 threshold. Details ▾Hide ▴
Reasoning

The ATM VCEP v1.5 BA1 threshold is grpmax Filtering AF >0.5% in gnomAD v4. The observed grpmax FAF is 1.327e-05 (0.001327%), far below 0.5%; therefore BA1 is not met.

Rule
Grpmax Filtering AF >0.5% in gnomAD v4 dataset
Evidence considered
  • ATM VCEP v1.5 specifies BA1 when grpmax Filtering AF is >0.5% in gnomAD v4.
  • gnomAD v4 reports grpmax FAF = 1.327e-05, with total AF = 1.858980237800752e-06 and 3/1,613,788 alleles observed; the maximum subpopulation frequency is 4.998833605492052e-05 in Admixed American individuals.
Sources
BS1 Not met: gnomAD v4 grpmax filtering allele frequency 0.001327% is below the 0.05% BS1 threshold. Details ▾Hide ▴
Reasoning

The ATM VCEP v1.5 BS1 threshold is grpmax Filtering AF >0.05% in gnomAD v4. The observed grpmax FAF is 1.327e-05 (0.001327%), below 0.05%; therefore BS1 is not met.

Rule
Grpmax Filtering AF >0.05% in gnomAD v4 dataset
Evidence considered
  • ATM VCEP v1.5 specifies BS1 when grpmax Filtering AF is >0.05% in gnomAD v4.
  • gnomAD v4 reports grpmax FAF = 1.327e-05 (0.001327%); the highest observed subpopulation AF is 4.998833605492052e-05 (0.00500%) in Admixed American individuals, also below the BS1 threshold.
Sources
BP4 Not met: SpliceAI maximum delta score 0.307 exceeds the ≤0.1 BP4 threshold. Details ▾Hide ▴
Reasoning

The ATM VCEP permits splice-impact assessment across variant types and assigns BP4_Supporting only when SpliceAI is <=0.1. The observed maximum delta score is 0.307, so the no-predicted-splice-impact requirement is not met. The laboratory missense-only BP4 override does not change this outcome because this is a nonsense, not missense, variant.

Rule
ATM VCEP v1.5 BP4 splicing sub-path: SpliceAI <=0.1.
Evidence considered
  • The governing ATM VCEP specification uses SpliceAI <=0.1 for BP4 splicing evidence and states that splice analysis should be considered for all variant types.
  • SpliceAI reports DS_AL 0.243 and DS_DL 0.307, with maximum delta score 0.307; this exceeds the BP4 no-impact threshold.

PS3 Not assessed: Sun et al. 2025 shows loss of function, but this assay is not among the three ATM VCEP-calibrated assays, so PS3 awaits expert review. Details ▾Hide ▴
Reasoning

The ClinGen HBOP ATM VCEP v1.5 framework states that PS3 must not be used at Strong strength; PS3_Moderate requires failure to rescue both an ATM-specific feature and radiosensitivity, while PS3_Supporting requires failure of an ATM-specific feature and cannot be based on radiosensitivity alone. The variant NM_000051.4:c.1039G>T (p.Glu347Ter) has a directly measured non-functional result in Sun et al. 2025 Table S1 (DeepATM_predicted=No, Confidence=High), indicating genuine prime-editing functional-screen evidence rather than a computational prediction. However, this assay is not one of the three ATM functional assays calibrated in the current VCEP specification, and the available evidence does not document the required ATM-specific phosphorylation and radiosensitivity readouts for this variant. Therefore PS3 cannot be assigned under the current VCEP without expert review of assay validity and calibration.

Rule
ClinGen HBOP ATM VCEP v1.5 PS3: do not use Strong; Moderate requires failure to rescue both an ATM-specific feature and radiosensitivity; Supporting requires failure to rescue an ATM-specific feature only and is not assigned for radiosensitivity alone.
Evidence considered
  • ClinGen HBOP ATM VCEP v1.5 specifies that PS3 is not used as Strong; PS3_Moderate requires failure to rescue both an ATM-specific feature and radiosensitivity, and PS3_Supporting requires failure of an ATM-specific feature and excludes radiosensitivity-only evidence.
  • Sun et al. 2025 Table S1 directly matches NM_000051.4:c.1039G>T / p.Glu347Ter and reports Classification=Non-functional, Confidence=High, and DeepATM_predicted=No; the latter indicates a direct prime-editing functional measurement rather than a model extrapolation.
  • The ATM VCEP functional-assay calibration document identifies its approved assays as the Mitui 2009, Barone 2009, and Scott 2002 assays and explicitly indicates that calibration is not per-variant evidence.
Still missing
  • No variant-specific result from one of the VCEP-approved ATM kinase-activity assays (PMID:18634022, PMID:19431188, or PMID:11805335) was identified.
  • The available Sun et al. result does not provide the VCEP-required paired ATM-specific feature and radiosensitivity readouts needed for PS3_Moderate, nor a VCEP-calibrated strength for this newer assay.
  • Human review is required to determine whether the direct prime-editing assay can be accepted and what PS3 strength, if any, is appropriate.

PS4 Not assessed: no qualifying case-control study was available for this variant. Details ▾Hide ▴
Reasoning

No qualifying case-control study was provided for ATM c.1039G>T (p.Glu347Ter).

Rule
ATM HBOP VCEP v1.5: Case-control studies; p-value ≤0.05 AND (odds ratio, hazard ratio, or relative risk ≥2 OR lower 95% CI ≥1.5). PS4_Moderate is not to be used.
Evidence considered
  • The ATM HBOP VCEP v1.5 permits PS4 only for case-control studies with p-value ≤0.05 and OR, HR, or RR ≥2, or lower 95% CI ≥1.5; it explicitly does not permit PS4_Moderate based on proband counting.
  • The exact-variant ClinVar submissions contain no validated publication trail or criterion lead, and the pre-extracted candidate paper PMID:17508274 does not mention ATM or c.1039G>T.
Still missing
  • A case-control study for this exact variant reporting p-value and an odds ratio, hazard ratio, or relative risk meeting the ATM VCEP PS4 threshold is needed.

PM3 Not assessed: no affected ataxia-telangiectasia proband, second ATM variant, or phase information was available. Details ▾Hide ▴
Reasoning

The applicable ClinGen HBOP ATM VCEP v1.5 specification evaluates PM3 for the autosomal-recessive ataxia-telangiectasia phenotype using weighted observations in unrelated affected probands, with the score determined by phenotype confidence, zygosity, and phase with a VCEP-classified pathogenic or likely pathogenic ATM variant. The case-evidence bundle contains no affected ataxia-telangiectasia proband observation for NM_000051.4:c.1039G>T, no documented second ATM variant, and no parental or offspring testing establishing trans or cis phase. Therefore, no PM3 points can be assigned.

Rule
ClinGen HBOP ATM VCEP v1.5: PM3 points are assigned per unrelated A-T proband (confident phenotype: 4.0 confirmed in trans, 2.0 phase unknown, 1.0 second variant unidentified or VUS, 2.0 homozygous; consistent phenotype values are half these values; homozygous observations are capped at two individuals). PM3 Supporting = 1 point, Moderate = 2 points, Strong = 4 points, Very Strong = at least 8 points.
Evidence considered
  • ClinGen HBOP ATM VCEP v1.5 specifies PM3 as a points-based criterion for ataxia-telangiectasia and requires assessment of affected-proband phenotype, zygosity, and phase with a VCEP-classified pathogenic or likely pathogenic ATM variant.
  • Review of the consolidated case-evidence bundle found no qualifying affected-proband, second-variant, parental, offspring, trans-phase, or cis-phase observation for NM_000051.4:c.1039G>T (NP_000042.3:p.(Glu347Ter)).
  • The available ClinVar classification and general ATM loss-of-function information do not establish PM3 because PM3 requires allelic observations in affected individuals rather than a final classification from another source.
Still missing
  • Affected-proband observations for the variant in individuals with ataxia-telangiectasia or an atypical/variant ataxia-telangiectasia phenotype are not documented.
  • A second ATM variant classified under the ATM VCEP specification is not documented.
  • Phase information from parental or offspring testing is not documented.
  • The phenotype confidence category and any qualifying laboratory results are not documented.

PP1 Not assessed: no pedigree or segregation data were available to demonstrate the variant co-segregates with disease. Details ▾Hide ▴
Reasoning

PP1 is applicable only through the ATM VCEP autosomal-recessive segregation pathway, which requires affected relatives to have both variants identified in the proband. The case-evidence bundle contains no usable family pedigree, affected-relative genotypes, parental/offspring testing, phase information, or meioses establishing segregation of NM_000051.4:c.1039G>T. Therefore no PP1 strength can be assigned.

Rule
AR condition only: affected relatives must have both variants identified in the proband; Supporting for 1 affected relative, Moderate for 2, Strong for at least 3 affected relatives.
Evidence considered
  • The ATM VCEP PP1 rule permits Supporting for segregation in 1 affected relative, Moderate for 2, and Strong for at least 3 affected relatives under the autosomal-recessive condition.
  • The ATM VCEP instructions require affected relatives to have both variants identified in the proband.
  • The consolidated case-evidence bundle provides no case-specific segregation, parental testing, affected-relative genotype, pedigree, or meiosis data for this variant.
Still missing
  • Documented affected relatives with phenotype consistent with the relevant ATM autosomal-recessive condition
  • Genotype results for the variant in each affected relative
  • Confirmation that affected relatives carry both variants identified in the proband
  • Pedigree and independent meioses sufficient to count segregation
Sources
BS3 Not assessed: available functional data show loss of function rather than rescue, so no variant-specific normal-function result supported BS3. Details ▾Hide ▴
Reasoning

The ClinGen HBOP ATM VCEP v1.5 framework defines BS3_Supporting when a variant rescues either an ATM-specific feature or radiosensitivity, and BS3_Moderate when it rescues both. The available direct functional result for NM_000051.4:c.1039G>T (p.Glu347Ter) is classified as non-functional, not functional or rescuing, in Sun et al. 2025 Table S1. Thus it does not support BS3. Because no variant-specific normal-function result from a VCEP-approved assay was identified, and the newer direct assay is not calibrated by the current VCEP, BS3 is not assessed rather than assigned as met or not_met on the basis of an unvalidated assay framework.

Rule
ClinGen HBOP ATM VCEP v1.5 BS3: Supporting requires rescue of either an ATM-specific feature or radiosensitivity; Moderate requires rescue of both.
Evidence considered
  • ClinGen HBOP ATM VCEP v1.5 specifies BS3_Moderate for rescue of both an ATM-specific feature and radiosensitivity, and BS3_Supporting for rescue of either one.
  • Sun et al. 2025 Table S1 directly matches NM_000051.4:c.1039G>T / p.Glu347Ter and reports Classification=Non-functional, Confidence=High, and DeepATM_predicted=No; this is evidence against normal function, not BS3 evidence.
  • The ATM VCEP functional-assay calibration document lists the approved assay systems but does not report a variant-specific result for c.1039G>T.
Still missing
  • No variant-specific normal-function or rescue result from a VCEP-approved ATM kinase-activity or radiosensitivity assay was identified.
  • The available direct prime-editing result is non-functional and therefore does not provide benign functional evidence.
  • Human review is required to determine whether any additional validated functional data exist and whether the newer assay can be incorporated under the VCEP framework.

BP2 Not assessed: no unaffected adult carrying the variant with a pathogenic ATM variant in trans, and no cis co-occurrence observation, was available. Details ▾Hide ▴
Reasoning

The applicable ClinGen HBOP ATM VCEP v1.5 specification evaluates BP2 using negative points for an unaffected, non-ataxia-telangiectasia individual carrying the variant with a VCEP-classified pathogenic or likely pathogenic ATM variant in trans, with age and clinical status requirements for the relevant downgrade scenario. The case-evidence bundle contains no unaffected adult observation with this variant and a qualifying ATM variant in confirmed or unknown phase, and no valid cis co-occurrence observation. Therefore, no BP2 points can be assigned.

Rule
ClinGen HBOP ATM VCEP v1.5: BP2 points per unaffected non-A-T individual are -4.0 for confirmed trans, -2.0 for phase unknown, and for homozygous observations -2.0 in a laboratory setting or -1.0 in a database setting, with homozygous observations capped at -2.0. BP2 Supporting = -1 point, Moderate = -2 points, Strong = at least -4 points.
Evidence considered
  • ClinGen HBOP ATM VCEP v1.5 specifies BP2 as a points-based criterion based on unaffected non-ataxia-telangiectasia individuals carrying the variant with a pathogenic or likely pathogenic ATM variant, with separate weights for confirmed trans, phase unknown, and laboratory/database settings.
  • Review of the consolidated case-evidence bundle found no qualifying unaffected adult, co-occurring ATM pathogenic/likely pathogenic variant, trans-phase observation, or cis-phase observation for NM_000051.4:c.1039G>T (NP_000042.3:p.(Glu347Ter)).
  • The variant's rare population frequency and reported classifications do not establish BP2 because BP2 requires an unaffected-individual allelic observation.
Still missing
  • An unaffected individual aged 18 years or older with no evidence of ataxia-telangiectasia carrying the variant is not documented.
  • A co-occurring ATM pathogenic or likely pathogenic variant classified under the ATM VCEP specification is not documented in such an individual.
  • Trans phase or cis phase for any co-occurring variant is not documented.
  • The number and setting of qualifying unaffected observations are not documented, so the laboratory/database point allocation cannot be determined.

PS1 N/ANot applicable: this nonsense change (p.Glu347Ter) is outside the PS1 missense and splice-region pathways. Details ▾Hide ▴
Reasoning

The variant is a nonsense change, NM_000051.4:c.1039G>T (NP_000042.3:p.(Glu347Ter)), not a missense substitution. The ATM VCEP PS1 missense pathway requires the same amino-acid change as an established pathogenic/likely pathogenic variant; the alternative ATM PS1 pathway is for splice-region variants with matched splice predictions. Neither pathway applies to this truncating variant.

Rule
ATM VCEP v1.5 PS1: use for missense changes when splicing is ruled out for both alterations; use the ATM PS1 Splicing table for splicing variants with similar predictions or observations of splice defect.
Evidence considered
  • The case normalization identifies NM_000051.4:c.1039G>T as NP_000042.3:p.(Glu347Ter), a nonsense variant.
  • The ATM VCEP PS1 specification applies PS1 to missense changes when splicing is ruled out, or to splice-region variants using its dedicated splicing table; it does not provide a PS1 pathway for a nonsense variant.

PS2 VCEPNot applicable: the ATM VCEP excludes PS2 because informative de novo occurrences have not been observed. Details ▾Hide ▴
Reasoning

The ClinGen HBOP ATM VCEP v1.5 specification makes PS2 not applicable for ATM autosomal-dominant and autosomal-recessive disease because informative de novo occurrences have not been observed and de novo events do not provide informative phase for autosomal-recessive disease.

VCEP rule
Not applicable per ClinGen HBOP ATM VCEP v1.5 for AD or AR disease.
Evidence considered
  • The ATM VCEP v1.5 rule states: do not use PS2 for AD or AR disease; informative de novo occurrences have not yet been observed and de novo AR conditions are unlikely to be informed by phase.
Sources
PM1 VCEPNot applicable: PM1 addresses missense variants in critical domains; this is a nonsense change. Details ▾Hide ▴
Reasoning

The variant is a nonsense change rather than a missense variant assessed for a critical functional domain. The applicable ATM VCEP framework marks PM1 as Not Applicable, and vcep_materials.json contains no ATM domain_tables entry. Therefore there is no authoritative approved-domain list to apply and PM1 is not applicable to this variant under the governing framework.

VCEP rule
ATM VCEP v1.5 PM1: Not Applicable.
Evidence considered
  • The ATM VCEP v1.5 framework lists PM1 as Not Applicable.
  • The consolidated vcep_materials.json has an empty domain_tables object for ATM.
  • The case variant is NP_000042.3:p.(Glu347Ter), a truncating nonsense variant, not a missense change for which a domain-based PM1 assessment would ordinarily be considered.
Sources
PM4 N/ANot applicable: PM4 is restricted to stop-loss variants; c.1039G>T is a nonsense change creating p.(Glu347Ter). Details ▾Hide ▴
Reasoning

The applicable ATM HBOP VCEP specification restricts PM4 to stop-loss variants. c.1039G>T is a nonsense variant that creates p.(Glu347Ter), not a stop-loss or in-frame protein-length change; PM4 is therefore not applicable.

Rule
ATM HBOP VCEP v1.5 PM4: use for stop-loss variants.
Evidence considered
  • Variant normalization identifies p.(Glu347Ter), a premature stop-gain, rather than loss of the native termination codon.
  • ATM HBOP VCEP v1.5 specifies PM4 for stop-loss variants.
Sources
PM6 VCEPNot applicable: the ATM VCEP excludes PM6 because informative de novo occurrences have not been observed. Details ▾Hide ▴

The ClinGen HBOP ATM VCEP v1.5 specification makes PM6 not applicable for ATM autosomal-dominant and autosomal-recessive disease because informative de novo occurrences have not been observed and de novo events do not provide informative phase for autosomal-recessive disease.

VCEP rule
Not applicable per ClinGen HBOP ATM VCEP v1.5 for AD or AR disease.
Evidence considered
  • The ATM VCEP v1.5 rule states: do not use PM6 for AD or AR disease; informative de novo occurrences have not yet been observed and de novo AR conditions are unlikely to be informed by phase.
Sources
PP2 VCEPNot applicable: the ATM VCEP marks PP2 not applicable, and this is a nonsense, not missense, variant. Details ▾Hide ▴
Reasoning

The governing ATM VCEP v1.5 framework marks PP2 as Not Applicable. In addition, the variant is a nonsense change rather than a missense variant for which a gene-level missense mechanism criterion would be assessed.

VCEP rule
ATM VCEP v1.5 PP2: Not Applicable.
Evidence considered
  • The ATM VCEP v1.5 framework lists PP2 as Not Applicable.
  • The case variant is NP_000042.3:p.(Glu347Ter), a nonsense variant rather than a missense substitution.
Sources
PP3 N/ANot applicable: PP3 splice-prediction scope covers silent, missense/in-frame, and non-canonical intronic variants, not exonic nonsense changes. Details ▾Hide ▴
Reasoning

ATM VCEP v1.5 limits PP3 splicing use to silent, missense/in-frame, and qualifying non-canonical intronic variants. This exonic nonsense variant, NM_000051.4:c.1039G>T (p.(Glu347Ter)), is outside that specified PP3 splice-prediction scope; its predicted protein-truncating consequence is not computational PP3 evidence.

Rule
ATM VCEP v1.5 PP3: SpliceAI >=0.2 for silent, missense/in-frame, and intronic variants outside donor/acceptor ±1,2 sites.
Evidence considered
  • The governing ATM VCEP specification assigns PP3_Supporting for SpliceAI >=0.2 only for silent, missense/in-frame, and intronic variants outside donor/acceptor ±1,2 sites.
  • The case SpliceAI maximum delta score is 0.307, but that score does not expand PP3 applicability to this nonsense variant.
Sources
PP4 VCEPNot applicable: the ATM VCEP forbids PP4 for both ATM-associated cancer predisposition and ataxia-telangiectasia. Details ▾Hide ▴
Reasoning

The ATM HBOP VCEP v1.5 specifies that PP4 must not be used for either autosomal-dominant ATM-related cancer predisposition or autosomal-recessive ataxia-telangiectasia.

VCEP rule
ATM HBOP VCEP v1.5: PP4 not applicable; do not use.
Evidence considered
  • For autosomal-dominant disease, breast cancer has multiple genetic etiologies and hereditary and sporadic causes cannot be readily distinguished; for autosomal-recessive disease, phenotype evidence is incorporated into the ATM PM3/BP2 table rather than used as separate PP4 evidence.

PP5 VCEPNot applicable: the ATM VCEP excludes PP5, and no ClinVar expert-panel pathogenic assertion exists for this exact variant. Details ▾Hide ▴
Reasoning

PP5 is not applicable under the ATM HBOP VCEP v1.5, and no exact-variant ClinVar expert-panel Pathogenic/Likely pathogenic assertion is present.

VCEP rule
ATM HBOP VCEP v1.5: PP5 not applicable. Additional group restriction: only an exact-variant ClinVar expert-panel Pathogenic/Likely pathogenic assertion could support PP5.
Evidence considered
  • ClinVar variation 1773580 exactly matches NM_000051.4:c.1039G>T (p.Glu347Ter), but all three Pathogenic submissions are ordinary single-submitter clinical laboratory assertions; the submission audit reports zero expert-panel submissions.
  • Aggregate ClinVar classification and ordinary laboratory submissions were not used to infer PP5.
Sources
BS2 VCEPNot applicable: the ATM VCEP designates BS2 as not applicable. Details ▾Hide ▴
Reasoning

The ATM VCEP v1.5 framework designates BS2 as not applicable. No BS2 assessment is made from the absence of homozygotes in gnomAD.

Evidence considered
  • ATM VCEP v1.5 lists BS2 as not applicable.
Sources
BS4 VCEPNot applicable: the ATM VCEP specifies BS4 as not applicable for both ATM-associated conditions. Details ▾Hide ▴
Reasoning

The ClinGen HBOP ATM VCEP v1.5 specifies BS4 as not applicable for ATM. For the autosomal-recessive ataxia-telangiectasia condition, informative lack-of-segregation instances are considered too rare to receive weight at this time; for autosomal-dominant ATM-related cancer predisposition, the VCEP cautions that low penetrance and phenocopies confound co-segregation analysis.

VCEP rule
Not applicable per ClinGen HBOP ATM VCEP v1.5; no BS4 weight for ATM.
Evidence considered
  • The ATM VCEP v1.5 BS4 rule is marked not applicable and states that informative lack-of-segregation instances in ataxia-telangiectasia families are too rare to be considered for weight at this time.
  • The VCEP also cautions that co-segregation analysis in the low-penetrance autosomal-dominant condition can produce false-positive results and that phenocopies may mimic lack of segregation.
Sources
BP1 VCEPNot applicable: the ATM VCEP marks BP1 not applicable, and this is a nonsense, not missense, variant. Details ▾Hide ▴
Reasoning

The governing ATM VCEP v1.5 framework marks BP1 as Not Applicable. The variant is also a nonsense change, not a missense variant to which a benign missense-mechanism criterion would apply.

VCEP rule
ATM VCEP v1.5 BP1: Not Applicable.
Evidence considered
  • The ATM VCEP v1.5 framework lists BP1 as Not Applicable.
  • The case variant is NP_000042.3:p.(Glu347Ter), a nonsense variant rather than a missense substitution.
Sources
BP3 VCEPNot applicable: the ATM VCEP forbids BP3, and this is a single-nucleotide nonsense change, not an in-frame indel. Details ▾Hide ▴
Reasoning

BP3 is explicitly not applicable in the ATM HBOP VCEP specification and instructs curators not to use it. Independently, this variant is a single-nucleotide nonsense change, not an in-frame insertion/deletion in a repetitive region.

VCEP rule
ATM HBOP VCEP v1.5 BP3: not applicable; do not use.
Evidence considered
  • ATM HBOP VCEP v1.5 lists BP3 as not applicable and states: Do not use.
  • Variant normalization identifies c.1039G>T as a nonsense single-nucleotide substitution, not an in-frame insertion or deletion.
Sources
BP5 VCEPNot applicable: the ATM VCEP specifies that BP5 must not be used. Details ▾Hide ▴
Reasoning

The ATM HBOP VCEP v1.5 specifies that BP5 must not be used.

VCEP rule
ATM HBOP VCEP v1.5: BP5 not applicable; do not use.
Evidence considered
  • The VCEP notes that multiple pathogenic variants can occur without a noticeable phenotypic difference and that ATM's low penetrance makes co-occurrence with other pathogenic variants insufficient for BP5.
Sources
BP6 VCEPNot applicable: the ATM VCEP excludes BP6, and no ClinVar expert-panel benign assertion exists for this exact variant. Details ▾Hide ▴
Reasoning

BP6 is not applicable under the ATM HBOP VCEP v1.5, and no exact-variant ClinVar expert-panel Benign/Likely benign assertion is present.

VCEP rule
ATM HBOP VCEP v1.5: BP6 not applicable. Additional group restriction: only an exact-variant ClinVar expert-panel Benign/Likely benign assertion could support BP6.
Evidence considered
  • ClinVar variation 1773580 exactly matches NM_000051.4:c.1039G>T (p.Glu347Ter), but it has no expert-panel submissions and no Benign/Likely benign assertion.
  • Aggregate ClinVar classification and ordinary laboratory submissions were not used to infer BP6.
Sources
BP7 N/ANot applicable: BP7 is limited to synonymous and qualifying deep-intronic variants; this is an exonic nonsense change. Details ▾Hide ▴
Reasoning

BP7 is restricted by the ATM VCEP to synonymous substitutions and qualifying deep intronic variants, with RNA-based BP7 available for observed absence of an aberrant RNA defect in silent or intronic variants. NM_000051.4:c.1039G>T is an exonic nonsense variant (p.(Glu347Ter)), so BP7 is not applicable.

Rule
ATM VCEP v1.5 BP7: synonymous and qualifying deep intronic variants; BP7(RNA) for observed lack of aberrant RNA defect in silent or intronic variants.
Evidence considered
  • The governing ATM VCEP specification limits BP7 to synonymous variants and deep intronic variants beyond +7 at donor sites or -21 at acceptor sites; RNA BP7 concerns lack of aberrant RNA defect for silent or intronic variants.
  • Case normalization identifies c.1039G>T as an exonic nonsense variant producing p.(Glu347Ter).

How the predictions were used

BP4 Not met: SpliceAI maximum delta score 0.307 exceeds the ≤0.1 BP4 threshold. Details ▾Hide ▴
Reasoning

The ATM VCEP permits splice-impact assessment across variant types and assigns BP4_Supporting only when SpliceAI is <=0.1. The observed maximum delta score is 0.307, so the no-predicted-splice-impact requirement is not met. The laboratory missense-only BP4 override does not change this outcome because this is a nonsense, not missense, variant.

Rule
ATM VCEP v1.5 BP4 splicing sub-path: SpliceAI <=0.1.
Sources
PP3 N/ANot applicable: PP3 splice-prediction scope covers silent, missense/in-frame, and non-canonical intronic variants, not exonic nonsense changes. Details ▾Hide ▴
Reasoning

ATM VCEP v1.5 limits PP3 splicing use to silent, missense/in-frame, and qualifying non-canonical intronic variants. This exonic nonsense variant, NM_000051.4:c.1039G>T (p.(Glu347Ter)), is outside that specified PP3 splice-prediction scope; its predicted protein-truncating consequence is not computational PP3 evidence.

Rule
ATM VCEP v1.5 PP3: SpliceAI >=0.2 for silent, missense/in-frame, and intronic variants outside donor/acceptor ±1,2 sites.
Evidence considered
  • The governing ATM VCEP specification assigns PP3_Supporting for SpliceAI >=0.2 only for silent, missense/in-frame, and intronic variants outside donor/acceptor ±1,2 sites.
  • The case SpliceAI maximum delta score is 0.307, but that score does not expand PP3 applicability to this nonsense variant.
