About PALB2

PALB2 encodes a tumor suppressor protein that partners with BRCA2 (and also interacts with BRCA1) to repair double-stranded DNA breaks through the homologous recombination pathway. It acts as a scaffold that stabilizes BRCA2 in the cell nucleus and helps recruit DNA-repair machinery to sites of damage. Inherited mutations in PALB2 increase susceptibility to breast cancer, with smaller associated risks for ovarian, pancreatic, and prostate cancer and melanoma, and inheriting two mutated copies causes Fanconi anemia complementation group N. Rare somatic alterations in PALB2 are also found across various tumor types.

Met Not met Not assessed Not applicable

The governing PALB2 VCEP is applicable.

PS1, PM1, PP2, and BP1 are not applicable under the PALB2-specific rules or variant-type restrictions.

PM5 is met at supporting strength under PALB2's truncation-specific rule because the p.Gln350HisfsTer11 premature termination codon is upstream of p.His1184 and the variant is assessed within the PVS1 Very Strong framework.

The PALB2 Version 1.3 VCEP makes PS2 and PM6 not applicable.

PP1 and BS4 are applicable in principle but cannot be assessed because the available evidence contains no family segregation, parental testing, meioses, or quantitative co-segregation result.

Among the requested criteria, PS4 cannot be assessed from the available exact-variant evidence because the only case-control enrichment estimate is for PALB2 truncating variants as a group; PP4, PP5, BP5, and BP6 are not applicable under the PALB2 VCEP specification.

All three requested criteria are not_applicable because NM_024675.4:c.1049_1050insT is a frameshift variant, outside the specified PP3/BP4/BP7 consequence scopes.

No predictor score was used to adjudicate an out-of-scope criterion.

Population evidence supports PM2 at Supporting strength because the variant is absent from all evaluated gnomAD datasets, including the available non-cancer subsets.

BA1 and BS1 are not met because no population frequency is observed above their PALB2 VCEP thresholds.

BS2 is not assessed because the available data contain no homozygote, unaffected-carrier, phenotype, or VCEP point-based evidence.

Met · 3

PVS1 Very strongMet, very strong: the early frameshift p.(Q350Hfs*11) truncates PALB2 near residue 360, far upstream of the VCEP p.His1184 cutoff. Details ▾Hide ▴
Reasoning

NM_024675.4:c.1049_1050insT is a frameshift producing NP_078951.2:p.(Q350Hfs*11), with a premature termination codon at approximately p.360 in the PALB2 coding sequence. PALB2 loss of function is an established disease mechanism, and the PALB2 VCEP specifies PVS1 for null variants while cautioning mainly for extreme 3-prime truncations or splice events that preserve the remainder of the protein. This early coding frameshift is on the VCEP default RefSeq transcript (NM_024675.3; the case uses the compatible .4 record), is not a canonical splice variant, and is far upstream of the VCEP p.His1184 cutoff; no evidence indicates NMD escape, a non-critical exon, or a repeat-region exception. The generic ClinGen SVI PVS1 framework therefore supports the default, full-strength call without a downgrade.

Rule
PALB2 VCEP PVS1 decision tree with generic ClinGen SVI PVS1 framework; no downgrade applies
Evidence considered
  • The PALB2 VCEP Version 1.3 identifies nonsense and frameshift variants as PVS1-eligible null variants when PALB2 loss of function is a known disease mechanism, with caution for extreme 3-prime truncations and splice events that leave the remainder of the protein intact.
  • The VCEP materials specify NM_024675.3/ENST00000261584.8 as the default RefSeq transcript; NM_024675.4 is the compatible version used for this case and yields p.(Q350Hfs*11).
  • The variant assessment gives a frameshift consequence and p.(Q350Hfs*11), placing the predicted premature termination around residue 360, well upstream of the PALB2 VCEP p.His1184 cutoff.
  • The ClinGen SVI PVS1 framework supports full-strength PVS1 for a loss-of-function frameshift when no applicable NMD-escape, non-critical-region, transcript-relevance, or other downgrade is established.
Sources
PM2 SupportingMet at supporting strength: the variant is absent from gnomAD v2.1, v4.1, and non-cancer subsets, giving observed frequency 0 versus the PALB2 threshold of 0.000333%. Details ▾Hide ▴
Reasoning

The PALB2 VCEP Version 1.3 specifies PM2 at supporting strength when the variant frequency is at most 1/300,000 (0.000333%) in the gnomAD subpopulation with the highest frequency, with an exception allowing a single observation confined to one subpopulation. The variant is absent from gnomAD v2.1 and v4.1, and also absent from the available gnomAD v2.1 and v3.1 non-cancer subsets. Thus the observed allele frequency is zero in every evaluated source and is below the PM2 threshold; the single-observation exception is not needed.

Rule
Frequency <=1/300,000 (0.000333%) in gnomAD subpopulation with the highest frequency; exception for >0.000333% when n=1 in one subpopulation only
Evidence considered
  • PALB2 VCEP Version 1.3 specifies PM2 supporting for frequency <=1/300,000 (0.000333%) in the gnomAD subpopulation with the highest frequency; it also permits a single observation confined to one subpopulation.
  • The variant is absent from gnomAD v2.1 and gnomAD v4.1, corresponding to observed frequency 0 in those datasets.
  • The variant is absent from the available gnomAD v2.1 non-cancer and gnomAD v3.1 non-cancer subsets as well.

PM5 SupportingMet, supporting: p.Gln350HisfsTer11 creates a premature stop near His360, upstream of the PALB2 VCEP cutoff p.His1184 and within its PVS1-Very Strong truncation rule. Details ▾Hide ▴
Reasoning

PM5 is met at supporting strength under the PALB2 VCEP truncation-specific rule. The rule applies to NMD-prone truncating variants that receive PVS1 at Very Strong strength and introduce a premature termination codon upstream of p.His1184. This variant is a frameshift producing p.(Gln350HisfsTer11), with the premature termination codon at approximately His360, well upstream of His1184; PALB2 loss of function is an established disease mechanism in the VCEP framework and the variant-level assessment identifies a frameshift with a suggested PVS1 Very Strong assessment. No same-residue missense comparator is required because PALB2's governing PM5 rule is truncation-specific rather than classic same-residue missense PM5.

Rule
PM5 Supporting: apply to NMD-prone truncating variants receiving PVS1 at Very Strong strength with premature termination codons upstream of p.His1184.
Evidence considered
  • The PALB2 VCEP PM5 rule applies at supporting strength to NMD-prone truncating variants receiving PVS1 at Very Strong strength with premature termination codons upstream of p.His1184.
  • The variant-level consequence is a frameshift, NP_078951.2:p.(Gln350HisfsTer11), placing the premature termination codon near His360, upstream of His1184.
  • The PALB2 VCEP PVS1 materials establish loss of function as a relevant disease mechanism, and the variant-level PVS1 assessment identifies this frameshift as eligible for the generic PVS1 framework with suggested Very Strong strength.

Not met · 2 · evaluated, threshold not reached

BA1 Not met: the variant is absent from gnomAD v2.1 and v4.1, so its frequency is not greater than the PALB2 VCEP BA1 threshold of 0.1%. Details ▾Hide ▴
Reasoning

The PALB2 VCEP Version 1.3 specifies BA1 when the gnomAD group maximum filtering allele frequency is greater than 0.1%. The variant is absent from gnomAD v2.1 and gnomAD v4.1; therefore no allele frequency exceeds the BA1 threshold. The available non-cancer subsets also report absence, but they do not alter the conclusion.

Rule
Grpmax Filtering AF >0.1% in gnomAD
Evidence considered
  • PALB2 VCEP Version 1.3 defines BA1 as group maximum filtering allele frequency >0.1% in gnomAD.
  • The variant is reported absent from gnomAD v2.1 and gnomAD v4.1, with no allele count or frequency reported.
  • The variant is also reported absent from the available gnomAD v2.1 and v3.1 non-cancer subsets.
Sources
BS1 Not met: the variant is absent from gnomAD v2.1 and v4.1, so its frequency does not exceed the PALB2 VCEP BS1 threshold of 0.01%. Details ▾Hide ▴
Reasoning

The PALB2 VCEP Version 1.3 specifies BS1 when the gnomAD group maximum filtering allele frequency is greater than 0.01%. The variant is absent from gnomAD v2.1 and gnomAD v4.1, so no observed frequency reaches the BS1 threshold. The available non-cancer subsets likewise report absence.

Rule
Grpmax Filtering AF >0.01% in gnomAD
Evidence considered
  • PALB2 VCEP Version 1.3 defines BS1 as group maximum filtering allele frequency >0.01% in gnomAD.
  • The variant is reported absent from gnomAD v2.1 and gnomAD v4.1, with no allele count or frequency reported.
  • The variant is also reported absent from the available gnomAD v2.1 and v3.1 non-cancer subsets.

PS4 Not assessed: group-level PALB2 truncating-variant enrichment was OR 4.69, but no exact-variant case-control statistic is available for c.1049_1050insT. Details ▾Hide ▴
Reasoning

The PALB2 VCEP specifies PS4 Strong for case-control studies requiring p-value ≤.05 and either odds ratio, hazard ratio, or relative risk ≥3 or lower 95% CI ≥1.5. PMID:28779002 reported a group-level OR of 4.69 (95% CI 2.27–9.68; p=6.9×10−6) for PALB2 truncating variants in 13,087 cases and 5,488 controls, but explicitly did not report NM_024675.4:c.1049_1050insT or p.(Q350Hfs*11). Because the available enrichment estimate is for a heterogeneous truncating-variant group rather than this exact variant, the evidence is insufficient to assess PS4 for this variant; no exact-variant case-control counts or statistics are available.

Rule
Case-control studies; p-value ≤.05 and (Odds ratio, hazard ratio, or relative risk ≥3 or lower 95% CI ≥1.5).
Evidence considered
  • The PALB2 VCEP PS4 rule is: "Case-control studies; p-value ≤.05 and (Odds ratio, hazard ratio, or relative risk ≥3 or lower 95% CI ≥1.5)."
  • PMID:28779002 studied 13,087 breast cancer cases and 5,488 controls and reported OR 4.69, 95% CI 2.27 to 9.68, p=6.9×10−6 for PALB2 truncating variants as a group, not for this exact variant.
  • The paper's exact-variant search found no report of c.1049_1050insT or p.(Q350Hfs*11), so its group-level result cannot be assigned as exact-variant PS4 evidence.
Still missing

PM3 Not assessed: no affected-proband observation or confirmed phase with a pathogenic PALB2 variant is documented for c.1049_1050insT. Details ▾Hide ▴
Reasoning

The PALB2 VCEP permits PM3 for the autosomal-recessive Fanconi anemia context and requires proband-level evidence of this variant with a pathogenic or likely pathogenic PALB2 variant in trans, with points assigned according to phenotype, classification, and phase. The available evidence does not document this exact variant in an affected Fanconi anemia proband, a pathogenic/likely pathogenic variant in trans or in cis, parental/offspring phase testing, or a qualifying homozygous observation. Therefore no PM3 points can be assigned and no PM3 strength can be established.

Rule
PALB2 VCEP PM3: assign Fanconi anemia PM3 points per proband; PM3_Supporting = 1 point, PM3 = 2 points, PM3_Strong = at least 4 points, and PM3_VeryStrong = at least 8 points.
Evidence considered
  • The PALB2 Version 1.3 specification states that PM3 is applicable for Fanconi anemia and assigns points per unrelated proband based on phenotype, the other variant's classification, and confirmed trans phase, homozygosity, or unknown phase.
  • The available case records identify NM_024675.4:c.1049_1050insT as a PALB2 frameshift, but provide no affected Fanconi anemia proband, second pathogenic PALB2 allele, or phase result for this exact variant.
  • The reviewed full-text extraction records for the five fetched papers do not report NM_024675.4:c.1049_1050insT or p.(Gln350HisfsTer11), and therefore provide no qualifying PM3 observation.
Still missing
  • Affected Fanconi anemia proband(s) carrying NM_024675.4:c.1049_1050insT
  • A pathogenic or likely pathogenic PALB2 allele identified in the same proband
  • Confirmed trans phase, or homozygosity, by parental, offspring, or equivalent phase testing
  • Fanconi anemia phenotype details sufficient to assign PALB2 VCEP PM3 points

PP1 Not assessed: no affected-relative segregation data, meioses, LOD score, or likelihood ratio are available to compare with the PP1 threshold of LOD ≥0.3 or LR ≥2:1. Details ▾Hide ▴
Reasoning

PP1 is applicable under the PALB2 Version 1.3 VCEP specification, which mandates quantitative co-segregation analysis. The available case evidence contains no identified affected relatives, parental transmission results, meioses, pedigree, LOD score, or Bayes factor for NM_024675.4:c.1049_1050insT. Therefore, segregation evidence cannot be established or assigned a PP1 strength. The VCEP thresholds are PP1 Supporting at LOD ≥0.3 or LR ≥2:1, Moderate at LOD ≥0.60 or LR ≥4:1, and Strong at LOD ≥1.26 or LR ≥18:1.

Rule
PP1: Quantitative PALB2 co-segregation is required; Supporting LOD ≥0.3 or LR ≥2:1, Moderate LOD ≥0.60 or LR ≥4:1, Strong LOD ≥1.26 or LR ≥18:1.
Evidence considered
  • The PALB2 VCEP requires quantitative co-segregation analysis for PP1 and gives Supporting, Moderate, and Strong thresholds of LOD ≥0.3/LR ≥2:1, LOD ≥0.60/LR ≥4:1, and LOD ≥1.26/LR ≥18:1, respectively.
  • No proband-specific pedigree, affected-relative testing, parental transmission, meiosis count, LOD score, or likelihood ratio was reported in the available case evidence.
Still missing
  • Obtain a pedigree and phenotype-confirmed affected-relative data.
  • Document variant testing and transmission status for each informative relative.
  • Calculate quantitative co-segregation evidence as a LOD score or Bayes factor/likelihood ratio.
Sources
BS2 Not assessed: no homozygote, unaffected-carrier, phenotype, or PALB2 VCEP BS2 point data are available for this absent variant. Details ▾Hide ▴
Reasoning

The PALB2 VCEP Version 1.3 requires BS2 point-based evidence from unaffected individuals or individuals with phenotypes inconsistent with PALB2-related disease, using its Fanconi anemia and cancer tables: supporting at 1 point, moderate at 2 points, and strong at 4 or more points. The population data available for this variant report only gnomAD absence and provide no homozygote observations, unaffected-carrier phenotypes, or BS2 points. BS2 therefore cannot be assessed.

Rule
BS2 supporting = 1 point; BS2 moderate = 2 points; BS2 strong = >=4 points, using the PALB2 Fanconi anemia and cancer BS2 tables
Evidence considered
  • PALB2 VCEP Version 1.3 assigns BS2 by points from specified unaffected or phenotype-inconsistent individuals: supporting at 1 point, moderate at 2 points, and strong at 4 or more points.
  • gnomAD v2.1 and v4.1 report the variant as absent and provide no homozygote, carrier-phenotype, or point-based BS2 observations.
Still missing
  • No homozygote or unaffected-carrier observations with phenotype information are available.
  • No PALB2 VCEP BS2 point calculation can be performed.

BS4 Not assessed: no affected non-carriers, non-segregation observations, LOD score, or likelihood ratio are available to compare with the BS4 threshold of LOD ≤−0.32 or LR ≤0.48. Details ▾Hide ▴
Reasoning

BS4 is applicable under the PALB2 Version 1.3 VCEP specification, which requires quantitative co-segregation analysis for lack of segregation. The available case evidence contains no affected relatives who tested negative for NM_024675.4:c.1049_1050insT, no phenotype-confirmed non-segregation observations, no meioses, LOD score, or Bayes factor. Therefore, BS4 cannot be assigned. The VCEP thresholds are BS4 Supporting at LOD ≤−0.32 or LR ≤0.48, Moderate at LOD ≤−0.64 or LR ≤0.23, and Strong at LOD ≤−1.28 or LR ≤0.053:1.

Rule
BS4: Quantitative PALB2 lack-of-segregation evidence is required; Supporting LOD ≤−0.32 or LR ≤0.48, Moderate LOD ≤−0.64 or LR ≤0.23, Strong LOD ≤−1.28 or LR ≤0.053:1.
Evidence considered
  • The PALB2 VCEP requires quantitative co-segregation analysis for BS4 and gives Supporting, Moderate, and Strong thresholds of LOD ≤−0.32/LR ≤0.48, LOD ≤−0.64/LR ≤0.23, and LOD ≤−1.28/LR ≤0.053:1, respectively.
  • The VCEP cautions that phenocopies and more than one pathogenic variant in a family can mimic lack of segregation; no family-level non-segregation data or quantitative result was reported in the available case evidence.
Still missing
  • Obtain phenotype-confirmed affected relatives who were tested for the variant.
  • Document reliable negative variant results and assess phenocopy or alternative-variant explanations.
  • Calculate quantitative non-segregation evidence as a LOD score or Bayes factor/likelihood ratio.

PS1 N/ANot applicable: the PALB2 PS1 specification is for matched splice-altering events, whereas this variant is a p.Gln350HisfsTer11 frameshift. Details ▾Hide ▴
Reasoning

PS1 is not applicable. The PALB2 VCEP PS1 specification is a splicing-table rule requiring comparison of a variant's predicted splicing event with a known or likely pathogenic splice-altering variant. This variant is a coding frameshift, p.(Gln350HisfsTer11), not a splice-site or splice-altering comparison supported by the PS1 table.

Rule
Use PALB2 PS1 Splicing table; prerequisite is a precisely matching predicted splicing event.
Evidence considered
  • The PALB2 VCEP framework lists PS1 as a splicing-table criterion and its table requires matching predicted splicing events.
  • The case variant is NM_024675.4:c.1049_1050insT with protein consequence NP_078951.2:p.(Gln350HisfsTer11), a frameshift rather than a splice-event comparison.

PS2 VCEPNot applicable: the PALB2 VCEP explicitly excludes PS2 because informative de novo occurrences have not been established for either relevant disease context. Details ▾Hide ▴
Reasoning

The PALB2 Version 1.3 VCEP specification explicitly designates PS2 as not applicable for both autosomal-dominant PALB2-related cancer predisposition and autosomal-recessive Fanconi anemia, because informative de novo occurrences are not established and de novo events are not expected to be informative for these conditions.

VCEP rule
PS2: Not applicable for PALB2 autosomal-dominant or autosomal-recessive disease under the Version 1.3 VCEP specification.
Evidence considered
  • The PALB2 VCEP Version 1.3 lists PS2 as not applicable for autosomal-dominant disease because informative de novo occurrences have not been observed.
  • The same specification lists PS2 as not applicable for autosomal-recessive disease because de novo occurrences are too rare to be informative and phase confirmation is generally required.

PS3 VCEPNot applicable: the PALB2 VCEP specification version 1.3 explicitly designates PS3 as not applicable. Details ▾Hide ▴
Reasoning

The governing ClinGen Hereditary Breast, Ovarian and Pancreatic Cancer Expert Panel specification for PALB2, version 1.3, explicitly designates PS3 as not applicable. Therefore, functional assay evidence cannot be used to assign PS3 for this variant under the applicable framework, regardless of the general ACMG/AMP definition of PS3.

VCEP rule
PALB2 VCEP version 1.3: PS3 is not applicable.
Evidence considered
  • The PALB2 ClinGen VCEP specification (version 1.3) lists PS3 with applicability marked Not Applicable.
Sources
PM1 VCEPNot applicable: the PALB2 VCEP explicitly marks PM1 not applicable, and no authoritative PALB2 domain table entry is present for residue 350. Details ▾Hide ▴
Reasoning

PM1 is not applicable under the governing PALB2 VCEP framework, which explicitly designates PM1 as not applicable. The authoritative vcep_materials.json domain_tables object is empty, so there is no approved PALB2 critical-domain list to apply to residue 350.

VCEP rule
PM1: Not Applicable in the PALB2 Version 1.3 VCEP framework.
Evidence considered
  • The governing PALB2 VCEP criteria mark PM1 as not applicable.
  • The authoritative domain_tables entry for PALB2 is absent or empty; therefore no approved critical-domain assignment can establish PM1.
Sources
PM4 VCEPNot applicable: the PALB2 VCEP explicitly designates PM4 as not applicable, and this variant is a truncating frameshift evaluated under PVS1. Details ▾Hide ▴
Reasoning

PM4 is explicitly designated Not Applicable in the PALB2 Version 1.3 VCEP specification. The variant is a truncating frameshift and is evaluated under PVS1 rather than PM4.

VCEP rule
PALB2 VCEP Version 1.3: PM4 not applicable
Evidence considered
  • The PALB2 Version 1.3 VCEP criteria list PM4 as Not Applicable.
  • The variant is a frameshift truncation, so its consequence is assessed through the PALB2 PVS1 framework rather than PM4.
Sources
PM6 VCEPNot applicable: the PALB2 VCEP explicitly excludes PM6 because assumed de novo occurrences are not informative for either relevant disease context. Details ▾Hide ▴
Reasoning

The PALB2 Version 1.3 VCEP specification explicitly designates PM6 as not applicable for both autosomal-dominant PALB2-related cancer predisposition and autosomal-recessive Fanconi anemia. The specification states that assumed de novo occurrences are not informative for these disease contexts.

VCEP rule
PM6: Not applicable for PALB2 autosomal-dominant or autosomal-recessive disease under the Version 1.3 VCEP specification.
Evidence considered
  • The PALB2 VCEP Version 1.3 lists PM6 as not applicable for autosomal-dominant disease because de novo is unlikely to be informative for common, apparently sporadic cancers.
  • The same specification lists PM6 as not applicable for autosomal-recessive disease because de novo occurrences are too rare to be informative and phase confirmation is difficult without parental testing.
Sources
PP2 VCEPNot applicable: the PALB2 VCEP marks PP2 not applicable, and this variant is a frameshift rather than a missense substitution. Details ▾Hide ▴
Reasoning

PP2 is not applicable under the governing PALB2 VCEP framework, which explicitly designates PP2 as not applicable. In addition, the variant is a frameshift rather than a missense variant for which a gene-level missense mechanism criterion would ordinarily be considered.

VCEP rule
PP2: Not Applicable in the PALB2 Version 1.3 VCEP framework.
Evidence considered
  • The governing PALB2 VCEP criteria mark PP2 as not applicable.
  • The normalized consequence is p.(Gln350HisfsTer11), a frameshift, not a missense substitution.
Sources
PP3 N/ANot applicable: this frameshift variant is outside PP3's calibrated missense/splicing scope, regardless of any available predictor score. Details ▾Hide ▴
Reasoning

Not applicable. NM_024675.4:c.1049_1050insT is a frameshift variant, predicted to cause p.(Gln350HisfsTer11). Under the governing PALB2 VCEP framework, PP3 is limited to predicted splicing impact for silent, missense/in-frame, and intronic variants outside donor/acceptor ±1,2 sites; the requested scope also excludes truncating and other protein-length-changing variants from PP3/BP4 computational assessment. Therefore no SpliceAI, REVEL, or other predictor score is evaluated for PP3.

Rule
PALB2 VCEP PP3: predicted splicing impact via SpliceAI ≥0.2 for silent, missense/in-frame, and intronic variants outside donor and acceptor 1,2 sites; not applicable to this frameshift.
Evidence considered
  • The case normalization identifies NM_024675.4:c.1049_1050insT as a frameshift producing p.(Gln350HisfsTer11).
  • The PALB2 VCEP specifies PP3 for predicted splicing impact in silent, missense/in-frame, and intronic variants, not for truncating variants.
Sources
PP4 VCEPNot applicable: the PALB2 VCEP excludes PP4 for heterogeneous autosomal-dominant breast cancer and directs recessive phenotype evidence to PM3. Details ▾Hide ▴
Reasoning

The PALB2 VCEP designates PP4 as not applicable for autosomal-dominant PALB2 disease because breast cancer has genetic heterogeneity and no clinical features readily distinguish hereditary from sporadic disease. For autosomal-recessive Fanconi anemia, the VCEP directs phenotype considerations to PM3 rather than PP4.

VCEP rule
Not applicable for PALB2 autosomal-dominant disease; for the autosomal-recessive disorder, use PM3 for specific phenotype considerations.
Evidence considered
  • The PALB2 VCEP states: "Do not use for AD disorder as breast cancer is a disease with multiple genetic etiology (genetic heterogeneity) and there are no features that can readily distinguish hereditary from sporadic causes."
  • The VCEP states that for the AR disorder, phenotype considerations should use PM3.
Sources
PP5 VCEPNot applicable: the PALB2 VCEP prohibits PP5, and ClinVar has no exact-variant expert-panel pathogenic or likely pathogenic classification. Details ▾Hide ▴
Reasoning

The PALB2 VCEP designates PP5 as not applicable and states that this criterion is not for use as recommended by the ClinGen Sequence Variant Interpretation VCEP Review Committee. ClinVar also returned no record for this exact variant, so there is no exact-variant expert-panel pathogenic or likely pathogenic classification to consider.

VCEP rule
Apply only to an exact-variant ClinVar expert-panel Pathogenic or Likely pathogenic classification; PALB2 VCEP guidance prohibits use.
Evidence considered
  • The PALB2 VCEP marks PP5 as "Not Applicable for this VCEP" and states: "This criterion is not for use as recommended by the ClinGen Sequence Variant Interpretation VCEP Review Committee."
  • The exact-variant ClinVar query returned no result; therefore no expert-panel Pathogenic or Likely pathogenic assertion is present.

The governing ClinGen Hereditary Breast, Ovarian and Pancreatic Cancer Expert Panel specification for PALB2, version 1.3, explicitly designates BS3 as not applicable. Therefore, functional assay evidence cannot be used to assign BS3 for this variant under the applicable framework, regardless of the general ACMG/AMP definition of BS3.

VCEP rule
PALB2 VCEP version 1.3: BS3 is not applicable.
Evidence considered
  • The PALB2 ClinGen VCEP specification (version 1.3) lists BS3 with applicability marked Not Applicable.
Sources
BP1 N/ANot applicable: the PALB2 BP1 rule applies to missense variants, while this variant is a p.Gln350HisfsTer11 frameshift. Details ▾Hide ▴
Reasoning

BP1 is not applicable because the PALB2 VCEP limits BP1 to missense variants. This variant is a coding frameshift, p.(Gln350HisfsTer11), so the benign evidence criterion does not apply.

Rule
BP1 Supporting: apply to all missense variants.
Evidence considered
  • The PALB2 VCEP BP1 rule states: apply to all missense variants.
  • The case variant has a frameshift consequence, p.(Gln350HisfsTer11), not a missense consequence.
Sources
BP2 VCEPNot applicable: the PALB2 VCEP explicitly excludes BP2 and provides no strength assignment for this criterion. Details ▾Hide ▴
Reasoning

The PALB2 Version 1.3 VCEP specification explicitly marks BP2 as not applicable and provides no usable strength rule for it. BP2 is therefore not assessed, regardless of any possible co-occurrence or phase observation.

VCEP rule
PALB2 VCEP BP2: Not applicable.
Evidence considered
  • The PALB2 Version 1.3 specification lists BP2 as not applicable and gives no applicable strength rule.
Sources
BP3 VCEPNot applicable: the PALB2 VCEP explicitly designates BP3 as not applicable, and this variant is a protein-truncating frameshift rather than a benign repeat-region indel. Details ▾Hide ▴
Reasoning

BP3 is explicitly designated Not Applicable in the PALB2 Version 1.3 VCEP specification. The variant is a protein-truncating frameshift, not an in-frame deletion or insertion in a repetitive region without functional impact.

VCEP rule
PALB2 VCEP Version 1.3: BP3 not applicable
Evidence considered
  • The PALB2 Version 1.3 VCEP criteria list BP3 as Not Applicable.
  • The variant produces p.(Q350Hfs*11), a protein-truncating frameshift, so BP3 is not applicable regardless of repeat-region context.
Sources
BP4 N/ANot applicable: this frameshift variant is outside BP4's calibrated missense/splicing scope, regardless of any available predictor score. Details ▾Hide ▴
Reasoning

Not applicable. NM_024675.4:c.1049_1050insT is a frameshift variant, predicted to cause p.(Gln350HisfsTer11). The governing PALB2 VCEP limits BP4 to no predicted splice impact for splicing variants and explicitly excludes missense variants; the requested scope further excludes frameshift and other protein-length-changing variants from PP3/BP4 computational assessment. Therefore no SpliceAI, REVEL, or other predictor score is evaluated for BP4.

Rule
PALB2 VCEP BP4: no predicted splice impact via SpliceAI ≤0.1 for splicing variants and not for missense variants; not applicable to this frameshift.
Evidence considered
  • The case normalization identifies NM_024675.4:c.1049_1050insT as a frameshift producing p.(Gln350HisfsTer11).
  • The PALB2 VCEP specifies BP4 for no predicted splicing impact in splicing variants and states that it is not applied to missense variants; the requested scope excludes truncating variants as well.
Sources
BP5 VCEPNot applicable: the PALB2 VCEP prohibits BP5 because co-occurring pathogenic variants are expected and do not reliably explain the phenotype. Details ▾Hide ▴
Reasoning

The PALB2 VCEP designates BP5 as not applicable and instructs users not to apply it because multiple pathogenic variants can co-occur without a noticeable phenotype difference, and PALB2's moderate penetrance makes alternate pathogenic variants more frequent.

VCEP rule
Do not use for PALB2.
Evidence considered
  • The PALB2 VCEP states: "Do not use: Cases with multiple pathogenic variants have been observed with no noticeable difference in phenotype ... PALB2 has moderate penetrance and will naturally occur with other pathogenic variants more frequently."
Sources
BP6 VCEPNot applicable: the PALB2 VCEP prohibits BP6, and ClinVar has no exact-variant expert-panel benign or likely benign classification. Details ▾Hide ▴
Reasoning

The PALB2 VCEP designates BP6 as not applicable and states that this criterion is not for use as recommended by the ClinGen Sequence Variant Interpretation VCEP Review Committee. ClinVar returned no exact-variant record, so no exact-variant expert-panel benign or likely benign classification exists.

VCEP rule
Apply only to an exact-variant ClinVar expert-panel Benign or Likely benign classification; PALB2 VCEP guidance prohibits use.
Evidence considered
  • The PALB2 VCEP marks BP6 as "Not Applicable for this VCEP" and states: "This criterion is not for use as recommended by the ClinGen Sequence Variant Interpretation VCEP Review Committee."
  • The exact-variant ClinVar query returned no result; therefore no expert-panel Benign or Likely benign assertion is present.

BP7 N/ANot applicable: this frameshift variant is not synonymous, so BP7 cannot be applied. Details ▾Hide ▴
Reasoning

Not applicable. NM_024675.4:c.1049_1050insT is a frameshift variant, predicted to cause p.(Gln350HisfsTer11), not a synonymous variant. BP7 is restricted to synonymous variants under the requested scope; consequently, no SpliceAI or RNA evidence is evaluated for BP7.

Rule
PALB2 VCEP BP7: use for synonymous and specified deep intronic variants; not applicable to this frameshift.
Evidence considered
  • The case normalization identifies NM_024675.4:c.1049_1050insT as a frameshift producing p.(Gln350HisfsTer11), not a synonymous change.
  • The PALB2 VCEP defines BP7 for synonymous and specified deep intronic variants; the requested scope limits BP7 to synonymous variants.
